I have enjoyed caring for you and your family over the years. With a lot of joy, and some sadness, I am writing to let you know of my intention to retire, and close my office June 30, 2020. I plan to remain in Indianapolis, and spend more time with my family and granddaughters. I would like to thank you for allowing me the privilege of participating in your medical care over the years. It has been a delight to meet and know so many wonderful people over the last 32 years.
No new patients will be scheduled beyond those already on the appointment calendar for 2020. Existing patients can continue to call and schedule appointments. No medical care will be provided after June 30, 2020, including requests for prescription refills. I will continue my nursing home practice, as well as teaching the dermatology residents at IU Medical Center.
I am in discussions with another Northside Indianapolis dermatology practice to take over your records. They would have access to your medical records to provide continuity of care if you desire. All patients who have been seen in the last two years will receive a letter in late April with contact information for this group. If you prefer, you can have your records sent to another provider. Any request before June 30, 2020 for records transfer should be sent to my office. Information on how to obtain your records July 1, 2020 and later from the new dermatology practice/record custodian will be included in my April letter, as well as posted on this website.
Once again, thank you for allowing me to be your doctor over the last 32 years.
