Dawes-Fretzin Dermatology has your medical records from Dr. Frost’s office practice, and will maintain them for 7 years from your last visit with Dr. Frost. Contact their office at 317-516-5000 or fax 317-516-5001 or website: dawesfretzin.com
Dr. Frost is currently only seeing patients in select nursing and rest homes. Those patients may contact him through their respective nursing or rest home nursing staff. All other patients from his office practice at 9290 Waldemar Road need to seek a new dermatologist.
No prescription requests, called in or electronic, will be filled, as the office is closed permanently. Dawes-Fretzin Dermatology will not be renewing prescriptions unless you have seen one of their providers first.
No medical advice or treatment will be provided after June 30, 2020 for patients previously seen in his office practice. Please contact your new provider. Dr. Frost is retired from his office practice.
